Most people who have ever stepped on a scale at a doctor's office have heard a number, been told which "category" it falls into, and left the room uncertain whether to celebrate, worry, or ignore it. The truth is that BMI is a useful but narrow tool, and the value of the number depends entirely on what you do with it. This article walks through the formula, the categories, the original purpose of the index, its well-known blind spots, the metrics that complement it, and the actions that genuinely improve health at any BMI. By the end, you should be able to look at a BMI result and know exactly what it does — and does not — tell you.

The formula in one line

BMI = weight in kilograms divided by height in meters squared. In metric, a 70 kg person who is 1.75 m tall has a BMI of 70 / (1.75 × 1.75) = 70 / 3.0625 = 22.86. In imperial units, the equivalent is BMI = (weight in pounds × 703) / (height in inches)². The same person at 154 lb and 69 inches computes as (154 × 703) / (69 × 69) = 108,262 / 4,761 = 22.74, the same number within rounding. The reason for the 703 is the conversion factor: 1 kg/m² equals about 703 lb/in². The formula is deliberately simple — it uses only weight and height, not body composition, age, sex, or ethnicity.

This simplicity is both the strength and the weakness. Strength: anyone can compute it in seconds with no equipment beyond a scale and a tape measure. Weakness: it cannot tell the difference between a kilogram of fat and a kilogram of muscle, between a kilogram of water and a kilogram of bone. Two people with the same BMI can have radically different body compositions, and the same person can shift their BMI by 2 points in a week simply by changing hydration or meal timing. Treat BMI as a starting point, not a verdict.

The WHO categories

The World Health Organization classifies adult BMI into fixed ranges: under 18.5 is "underweight," 18.5 to 24.9 is "normal weight," 25.0 to 29.9 is "overweight," and 30.0 or above is "obese." Obesity is often split into Class I (30.0–34.9), Class II (35.0–39.9), and Class III (≥40.0). These cutoffs were chosen in the 1990s based on large epidemiological studies linking BMI to mortality risk, with the lowest mortality seen in the 22.5–25.0 range. They were never designed to be diagnostic for individuals; they were designed to help compare risk across large populations.

The categories are useful for one specific purpose: identifying people at statistically higher risk of weight-related disease, so that further assessment can be offered. They are not useful for labeling individuals, judging appearances, or dictating goals without context. A BMI of 26 in a muscular athlete is not the same as a BMI of 26 in a sedentary person with low muscle mass, even though the number is the same.

Why BMI was designed for populations, not individuals

The index was developed in the 19th century by Adolphe Quetelet, a Belgian mathematician, as a tool for studying the average physique of large groups — soldiers, prisoners, entire populations. Quetelet was explicit that it was not meant to characterize individual bodies. The name "Body Mass Index" itself was popularized in 1972 by Ancel Keys, who used it precisely because it was a quick, cheap proxy for body fat in epidemiological research. The whole point was to have a single number that could be calculated for thousands of people and used to find statistical patterns. Applying it as a personal verdict is a use case its designers never intended.

The good news is that BMI does work at the population level. Across millions of people, average mortality rises steadily as BMI moves away from the 22.5–25.0 range, in a J-shaped curve with a small uptick at the underweight end and a steep climb at the obese end. The bad news is that any individual can be far from the population average for reasons that have nothing to do with health: muscle mass, frame size, hydration, recent illness, pregnancy. So the question is never "What is my BMI?" in isolation; it is "What is my BMI, and what other data points refine the picture?"

The body-fat vs muscle confusion

A 90 kg professional rugby player who is 1.85 m tall has a BMI of 26.3 — "overweight" by WHO standards. His body fat percentage, measured by DEXA scan, is often 12–15%, comparable to a sedentary person of normal BMI. The misclassification happens because muscle is roughly 18% denser than fat, so a kilogram of muscle occupies much less space. The mirror image is also possible: a 65 kg person at 1.70 m has a BMI of 22.5 — "normal" — but a body fat percentage of 35% if most of their weight is fat rather than muscle. This is the classic "skinny-fat" pattern, where the BMI looks fine but metabolic risk is elevated.

The practical lesson: BMI is a proxy for body mass, not for body composition. The two diverge most at the extremes — very muscular athletes, very sedentary older adults, people with significant edema, and people recovering from illness. For everyone else, BMI is a reasonable but imperfect signal. Treat it as a conversation starter with a healthcare professional, not as a stand-alone diagnosis.

Better follow-up metrics

Several measurements refine what BMI alone cannot tell you. Waist circumference, taken at the level of the navel, is a strong independent predictor of metabolic risk: above 102 cm (40 in) for men and 88 cm (35 in) for women signals elevated risk regardless of BMI. Waist-to-height ratio (waist divided by height) is even simpler: aim to keep your waist under half your height. A 1.80 m adult should keep their waist under 90 cm; a 1.60 m adult, under 80 cm. This rule is sometimes called "keep your waist less than half your height."

Body fat percentage, when measured accurately, is the most informative single number. DEXA scans are the gold standard (about $50–$150 in many cities), with high accuracy and the bonus of a regional breakdown. Bioelectrical impedance scales are cheap and convenient but vary with hydration; a single reading is not reliable, but consistent readings at the same time of day can show trends. Skinfold calipers, used by trained practitioners, give reasonable estimates for under $20 worth of equipment and a little practice. For most people, waist circumference plus a sensible BMI is enough to start.

A blood panel — fasting glucose, HbA1c, lipid profile, and (where relevant) liver enzymes — is the most useful additional step. These numbers tell you about metabolic health directly, instead of inferring it from a height-weight ratio. Two people with the same BMI can have very different blood markers; the markers are what actually matter for long-term risk.

The Asian-population adjustment

In 2000, the World Health Organization noted that Asian populations tend to develop weight-related metabolic complications at lower BMIs than people of European descent. The cutoffs for "increased risk" were revised for Asian adults: "overweight" begins at 23.0 rather than 25.0, and "obese" begins at 27.5 rather than 30.0. China, Japan, Singapore, and South Korea have adopted these or similar adjusted thresholds in their public health guidance. The reason is partly genetics (different body fat distribution at the same BMI) and partly lifestyle (different typical diets and physical activity patterns).

For an individual of Asian descent, the practical implication is to interpret BMI 23.0–24.9 not as "fine, ignore it" but as a yellow zone worth a closer look at waist circumference, physical activity, and blood markers. This is especially important given rising rates of type 2 diabetes in many Asian countries, where the disease often appears at BMIs that Western guidelines label as "normal."

BMI in children, pregnancy, and what to actually do with the number

In children and adolescents (typically 2–20 years), BMI is calculated the same way but interpreted using age- and sex-specific percentiles rather than the fixed adult cutoffs. A BMI at the 85th percentile is "overweight" and at the 95th is "obese." Growth curves are essential, because a child's BMI naturally rises and falls as they mature. In pregnancy, BMI is used to set weight-gain targets: underweight women are advised to gain 12.5–18 kg, normal-weight 11.5–16 kg, overweight 7–11.5 kg, and obese 5–9 kg. The categories are the same as adult cutoffs but the action that follows is very different.

For adults, the most useful thing to do with a BMI number is to combine it with two or three other data points: waist circumference, a recent blood panel, and a realistic assessment of your physical activity and eating patterns. If the picture is consistent and healthy, BMI alone is a useful but minor data point. If BMI says "overweight" but waist is fine and blood markers are excellent, you are probably healthy regardless. If BMI is "normal" but waist is high and blood markers are concerning, the BMI label is misleading. The number is a clue, not a sentence.

The actions that improve health at almost any BMI are well established: regular physical activity (150 minutes/week of moderate exercise is a common baseline), a diet rich in vegetables, fruit, whole grains, and legumes, enough sleep (7–9 hours for most adults), minimal tobacco and excessive alcohol, and routine medical check-ups. These are unglamorous and not specific to a BMI value, which is exactly why they work across the population. No number on a scale substitutes for them.
